Your largest customer sends a vendor resilience questionnaire. Your insurer asks for continuity evidence at renewal. A contracting officer wants your COOP documentation. Most organizations discover at that moment that they have the capability but not the proof — or neither.
This scorecard measures 36 controls across six domains and returns a scored profile, a prioritized remediation sequence, and a report you can hand to whoever asked. It takes about eight minutes.
The question behind the question
Resilience used to be a discretionary program. It is now a condition of doing business, and it arrives as a request for documentation with a deadline attached. These are the four moments that send organizations looking for a plan they assumed they already had.

Cyber and business interruption underwriters now price on demonstrated controls. Immutable backups and tested restores move the premium. Assertions without evidence do not.
Continuity of operations documentation is a routine requirement in solicitation response. Assembling it after the RFP drops costs you the schedule you needed for the technical volume.
Ransomware, a failed vendor, a key departure, a regional weather event. The plan gets tested whether or not you tested it first, and the difference shows up in the first ninety minutes.
The gap is almost never willingness. It is that resilience work spans IT, operations, legal, HR, and finance, so it belongs to everyone and therefore to no one. The scorecard exists to make the gaps specific enough that someone can be assigned to close them.
How it works
Six domains, six controls each. Each control is rated on a four-point maturity scale. "Not sure" is a valid answer and is recorded as a visibility gap, because an unanswerable question is itself a finding.
A weighted score per domain and an overall maturity tier, plotted against a reference baseline. Load-bearing controls carry 1.5× weight, so the score reflects exposure rather than checkbox count.
Every control below "documented and current" becomes a finding with a specific corrective action, ranked by weight times shortfall. Export the whole thing as a report you can take to a leadership meeting.
